Student Questionnaire – 2006
Eligible students who wish to be considered for this program must complete this form and send to Michelle Gautreau via fax at 603-885-1454. The student questionnaires are usually due by the third week in September. Please contact your guidance office or Michelle Gautreau at 603-885-4402 for the specific deadline.
Photo Release Form
This form must be completed by all students accepted into the program. The student must bring the original to the first orientation session.
